The offices of Family Legacy Counseling are located at 5415 NW 88th St, Suite 100 in Johnston, Iowa . If you are traveling on I80/35 you will want to take the 86th Street exit and turn north. Turn west onto NW 54th Ave, then turn north onto NW 88th St. Click here for a map to our location.

Office Hours
Family Legacy Counseling office is open Monday through Friday 8:00 a.m. to 4:00 p.m. During the times our receptionist is unavailable to take your call, we have voice mail for your convenience.

Fees
Payment is due at the time of service. Our fees range from $180 to $90 per session. We are accepting the following insurances: Wellmark Blue Cross/Blue Shield, United Health Care, John Deere Health and Midlands Choice.
